Surety/Bonds forms for new contractors:

If you are a firm that will need to post bid bonds, provide construction owners with performance and payment bonds, or subdivision bonds, please submit the following required information:

• Past three years of CPA-prepared fiscal year-end reviewed statements
• Current personal financial statements for all owners of the company (please request statement from your bank or contact our office)
• Current work-in-progress schedule
